MY FIRST VISIT
New Patient Forms
On your 1st visit to our office you will be welcomed as a member of our family and our Team will greet you with a warm smile. You will then be asked to fill out our required forms, which will help us gain some information about yourself and your condition. The forms are located in our New Patient Link.
Consultation
A member of our team will then talk with you and ask you some specific questions regarding your condition. You will then watch a short video introducing you to our office and an explanation of what Dr. Rosales will be explaining and performing. If Dr. Rosales can help you, the doctor will let you know. If Dr. Rosales cannot help you, you will be referred to a health professional that will be able to help you the most.
Examination
After a thorough consultation with the doctor, you will have a complete chiropractic examination. This will include testing your reflexes, your range of motion as well as other standard orthopedic, neurological, postural, and physical examinations. Dr. Rosales will gain valuable information regarding your condition so that a better understanding of how chiropractic can help you. If necessary, the doctor may take X-rays or refer you for additional testing procedures.
Radiographic Images
X-rays are a standard practice in medicine to get a full evaluation of a patient’s internal structures. The need for X-ray is considered on a case-by-case basis.
Why x-rays you ask:
1st To provide structure records of your spine,
2nd To rule out any disease or pathologies,
3rd To locate fractures or other signs of trauma,
4th To identify of long term spinal decay, and
5th To confirm other examination findings.
Our equipment is considered to be “low radiation exposure dosage” for diagnostic imaging. Only those x-rays that are truly indicated will be performed.
This day is part of the “information gathering” that is necessary to give you the highest quality of care. You will not receive your 1st adjustment today (except in some emergency cases). Please do not hesitate to ask questions as we complete today’s visit.

My Second Visit
Report of Findings
There is a short 2nd day video that will give you background information that is necessary in order to understand the information given to you at your report of findings. Dr. Rosales will give you a report of findings including the examination and x-rays. He will give you the best recommendations for your initial course of treatment. Nothing will be done in our office without your consent.
